Zepbound® is one of the most effective weight loss medications available today, with clinical trials showing people lost an average of about 21% of their body weight over time. The medication, tirzepatide, is taken as a once-weekly injection under the skin. But depending on how you get your prescription—and how you pay for it—you may receive Zepbound® in a different type of injection device.

Zepbound® comes in three delivery formats: a single-dose injector pen, the multi-dose Zepbound® KwikPen®, and a single-dose vial used with a syringe. All contain the same medication and are injected under the skin. The biggest differences are how the medication is packaged, how each dose is prepared, how the injection is delivered, and has access to each option.

For most people with commercial insurance—or those paying full retail price—Zepbound® comes as a single-dose pen that’s used once and discarded. People paying cash through LillyDirect® can choose either the four-dose KwikPen® or a single-dose vial. The KwikPen® is also the delivery device used when you get Zepbound® through Medicare’s GLP-1 Bridge Program.

What is the Zepbound® KwikPen®?

The Zepbound® KwikPen® is a reusable injection pen that contains four premeasured weekly doses of tirzepatide—enough for one month of treatment in a single device. Unlike Zepbound’s original single-dose pen, you’ll use the same KwikPen® for four injections, attaching a new disposable pen needle each week.

The KwikPen® became available for Zepbound® in the U.S. in 2026. It joined the existing single-dose vial as a cash-pay option through LillyDirect®, giving people without insurance coverage another way to purchase the medication. Meanwhile, the original single-dose injector pen remains the standard option for most people using commercial insurance or paying full retail price. The KwikPen® is also the only Zepbound® delivery device available through Medicare’s GLP-1 Bridge Program.

Zepbound® KwikPen®: How to use it

Each KwikPen® holds four fixed doses—one dose per week, for a full month of treatment in a single device. While you should refer to Lilly’s how-to-use guide before each use, here’s an overview of how the process typically goes:

  1. Wash your hands and gather your supplies: the KwikPen®, a new pen needle, an alcohol swab, gauze or a cotton ball, and a sharps disposal container.
  2. Check the pen. Pull off the cap and inspect the label, dose strength, and expiration date. The medicine should be clear and colorless to slightly yellow.
  3. Attach a new pen needle for every injection. Push it on straight and twist until it’s tight, then remove both the outer and inner needle shields. Pen needles aren’t included with the KwikPen® and are sold separately—more on which type to get below.
  4. Prime the pen. Turn the dose knob until you hear two clicks and see an extended line in the window. Hold the pen with the needle pointing up, tap the cartridge to clear air bubbles, then push the dose knob in and hold for a slow count of five. You should see a drop of medicine at the needle tip.
  5. Dial your dose. Turn the dose knob until it stops and the “1” appears in the window.
  6. Choose where to inject. Pick a spot on your stomach, thigh, or the back of your upper arm—no site works better than another, so go with whatever’s easiest to reach. Rotate between spots week to week even if you use the same general area to lower the chance of injection-spot reactions. 
  7. Clean the injection site. Wipe the area with the alcohol swab and let it air-dry completely before injecting.
  8. Inject. Insert the needle at a 90-degree angle into your stomach, thigh, or the back of your upper arm, then push the dose knob in until it stops. Hold for a slow count of five before removing the needle. If a small drop of blood appears, press the gauze or cotton ball gently against the site for a few seconds.
  9. Dispose of the needle in a sharps container right away. Replace the outer shield on the pen and store it for next week.

What happens with the KwikPen® after 4 doses?

After 4 weekly doses, the pen mechanically stops you from dialing a full dose again—a built-in safeguard against injecting a partial amount. Discard the pen at that point even if it looks like medicine remains; the leftover volume isn’t enough for a complete dose.

Zepbound® KwikPen®: Safe storage tips

Before first use, keep the KwikPen® refrigerated between 36°F and 46°F, the same as vial formats. Once you’ve used it for the first time, the KwikPen® has more flexibility than a single-dose pen or vial: it can stay at room temperature (up to 86°F) for up to 30 days, or until you’ve given all four weekly doses, whichever happens first, according to Noom’s guide to Zepbound® refrigeration. Freezing isn’t reversible—a KwikPen® that’s been frozen needs to be discarded and replaced even if the liquid still looks normal, since temperature damage to tirzepatide isn’t visible.

How to travel with Zepbound® KwikPen®

That 30-day, room-temperature window may make the KwikPen® more practical for travel than handling four separate vials or single-dose pens. A few situations worth planning for:

  • Day trips and errands: Don’t leave the KwikPen® in a hot car, glove compartment, or a bag sitting in direct sun—car interiors heat past 86°F faster than it feels outside. Keep it with you instead.
  • Hotels and rentals: Mini-fridges can run colder than expected near the back wall or freezer compartment, which risks freezing the pen. If refrigeration seems unreliable, room-temperature storage in the original carton is often the simpler option, as long as you’re still within the 30-day/4-dose window.
  • Flying: Pack the KwikPen® in your carry-on rather than checked luggage, in its original labeled packaging. TSA allows injectable medications through security even above standard liquid limits, and changes in cabin altitude aren’t expected to affect the medication. If you’re also carrying pen needles, keep them with the KwikPen and mention them at the security checkpoint.
  • International travel: Bring your prescription information and keep the KwikPen in its original packaging, since some countries have their own rules about carrying prescription medication across borders.

Zepbound® KwikPen®: Will I need to attach a needle?

Yes, and here’s why: the KwikPen® works a little differently than Zepbound’s original single-dose pen, which comes with a built-in needle. The KwikPen® doesn’t include pen needles, so you’ll need to pick up compatible disposable ones separately. Not sure which to get? One thing to keep in mind: even though your KwikPen® is designed to last for four weekly doses, you’ll attach a brand-new needle before every single injection. Everyone starts at the lowest 2.5 mg dose and moves up roughly every four weeks, as tolerated, up to a maximum of 15 mg—a gradual approach designed to limit nausea and other GI side effects while your body adjusts. The Medicare GLP-1 Bridge program

The Zepbound® KwikPen® is the only Zepbound® format covered under the Medicare GLP-1 Bridge program, a temporary Medicare program set to run through December 2027. Eligible Medicare Part D beneficiaries pay a flat $50 copay for the KwikPen® regardless of dose strength. Zepbound®’s single-dose pens and vials aren’t included in the Bridge program—only the KwikPen® qualifies.

  • Cap won’t come off: Gently twist it back and forth, then pull it straight off.
  • Dose knob is hard to push: Try pushing more slowly. If that doesn’t help, the needle may be blocked—attach a new pen needle and prime the pen again. If you notice dust, food, or liquid inside the pen, discard it and start a new one.
  • Pen won’t let you dial to a full dose: This means there isn’t enough medicine left for a complete dose. Discard the pen, including any leftover medicine—don’t try to inject what’s left.
  • Pen looks or acts damaged: If the plunger won’t advance, the medicine looks cloudy, or the device otherwise seems faulty, don’t attempt repairs or use it. Dispose of it safely, use a replacement pen, and contact Lilly at 1-800-LillyRx (1-800-545-5979) to report the issue.
