  • 2.5 mg: Zepbound® treatment begins with a 2.5 mg once-weekly dose for the first 4 weeks. This phase is meant to help your body adjust and reduce side effects. It’s not intended for significant weight loss, but many do lose some weight, even at this level.
  • 5 mg: After the adjustment period, the dose typically increases to 5 mg weekly. At this level, studies have shown that, on average, a person can lose about 15% of their body weight. 
  • 10 mg: At this moderate dose, people experienced an average weight loss of 20% over 72 weeks. About 89% lost at least 5%.
  • 15 mg: The highest available dose delivered the greatest results, 21% average weight loss. 91% lost at least 5%.

What influences your dose path with Zepbound®?

  • How you’re feeling: If side effects like nausea or fatigue are bothering you, your provider may pause, reduce, or slow down dose increases to give your body more time to adjust.

What are signs it’s time to adjust your dose of Zepbound®?

Your dose will be adjusted based on how your body is responding. Here are a few scenarios:

  • If side effects linger or get in the way of daily life, staying at your current dose longer—or even dialing it back—might help.
  • If your health changes, including new medications or conditions, your dose may need to be reviewed to keep things safe and effective.

Do higher doses of Zepbound® cost more?

It depends on which type of Zepbound® you’re using. Zepbound® comes in two different forms: a prefilled injection pen and a vial and syringe. The vial and syringe can be a more affordable way for people to access the medication, as insurance coverage is unlikely for weight loss medications. 

Let’s go through both.

  • Prefilled pen: If you’re using the prefilled auto-injector pens, the monthly cost stays the same, around $1,086 without insurance, regardless of your dose. Each pen is priced as a single unit. 
  • Vial and syringe: Pricing can vary by dosage for those using single-dose vials. Eli Lilly offers a self-pay option through LillyDirect®, where vials are priced at $349 for the 2.5 mg dose and $499 for the 5 mg, 7.5 mg, and 10 mg doses. If your dose needs to be increased beyond 10 mg, you’ll need to switch to the pen format, which comes at a higher monthly price.

Will there ever be a Zepbound® (tirzepatide) pill?

Right now, Zepbound® is only available as a weekly injection. It’s the standard method for delivering tirzepatide for weight management, and there’s no FDA-approved pill version just yet.

Eli Lilly—the manufacturer of Zepbound®—is developing a once-daily pill called orforglipron, an oral GLP-1 receptor agonist. In early clinical trials, orforglipron helped people lose an average of about 15% of their body weight over 36 weeks.

Orforglipron is now in Phase 3 trials to evaluate its long-term safety and effectiveness. If results continue to look good, a pill form of GLP-1 therapy could become available in the future.

Currently, the only FDA-approved GLP-1 pill is Rybelsus®. It contains the active ingredient semaglutide, like Wegovy® and Ozempic®, and is approved for treating type 2 diabetes. 

How to take Zepbound®: Tips for getting the best weight loss results

Taking Zepbound® consistently each week is one of the best ways to stay on track with your weight loss goals and manage any potential side effects. Here’s how to make your dosing routine work for you.

How to use Zepbound® prefilled pensThe prefilled pen is designed to make the process simple, even if you’re new to injections. Here’s a step-by-step guide:
1. Take your pen out of the fridge and check the medication inside. It should be clear or slightly yellow, never cloudy or frozen.
2. Let the medication come to room temperature to reduce the chances of injection site irritation.
3 Wash your hands and prep your injection site. Remember that tirzepatide should always be injected just beneath the skin, not into a muscle or vein. Clean the spot with an alcohol swab and let it dry before injecting.
4. Pull off the gray base cap. Don’t twist it, and don’t try to recap the pen afterward—it’s designed for one-time use.
5. Place the clear base flat against your skin. Make sure it’s firmly in place.
6. Turn the lock ring to unlock the pen.
7. Press and hold the purple injection button. You’ll hear two clicks: The first click means the injection has started. The second click means it’s done.
8. Hold the pen in place for up to 10 seconds to make sure you get the full dose.
9. Check the pen window—you’ll see the gray plunger if the dose was delivered.
10. Dispose of the pen in a proper sharps container.
How to use Zepbound® single-dose vialsZepbound® also comes in single-dose vials, which are injected using a standard syringe and needle. Here’s how to do it safely and effectively:
1. Gather your supplies. You’ll need your Zepbound® vial, a syringe and needle, an alcohol swab, gauze or cotton ball, and a sharps container.
2. Inspect the vial. Take it out of the fridge and make sure the medication is clear to slightly yellow, with no particles.
3. Wash your hands and prep your injection site. Just like with the pen, choose an area with a bit of soft tissue. Wipe the spot with an alcohol swab and let it air dry before you inject.
4. Fill the syringe. Wipe the vial top, insert the needle, inject some air into the vial to help draw the liquid out, and then draw up the medication. Check for air bubbles and tap them out.
5. Inject the dose. Pinch the skin, insert the needle, and slowly press the plunger. Hold for a few seconds, then remove the needle.
6. Dispose properly. Toss the used syringe and needle into a sharps container right away.
How to storeUnused Zepbound® pens should be kept in the fridge (between 36°F and 46°F). They can stay at room temperature (up to 86°F) for 21 days at most. Never freeze a pen—if it freezes or looks damaged, toss it.

Zepbound® vials should be stored just like the pens—keep refrigerated, never freeze, and discard properly after use. 
How to minimize side effectsWhile research hasn’t confirmed a perfect formula for avoiding side effects, many people find that small adjustments make a big difference. Consider the following tips:

Evening injections: Taking your dose at night might let you sleep through mild side effects like nausea.

Weekend dosing: Choosing a less busy day gives you time to rest if needed.

Avoid heavy meals: Skip rich or spicy foods around your injection time to prevent stomach upset.

Stay hydrated: Drinking plenty of fluids before and after dosing can help with digestive side effects like constipation or indigestion.

What happens if you miss a dose?

Missing a Zepbound® dose can happen. The key is knowing how to respond in a way that keeps your treatment safe and effective. Follow these guidelines to manage a missed dose without disrupting your progress:

Zepbound® dosage: Side effects and warnings as your dose increases

As your Zepbound® dose gradually increases, it’s common for some side effects to return, especially during the week following a dose change. That’s why your provider increases your dose slowly, to help your body adjust and minimize discomfort.

If you do notice side effects, many tend to fade as your body gets used to the new dose. But if something doesn’t feel right, always let your provider know. They can pause your dose increases, make adjustments, or offer ways to ease the symptoms.

Here are some common side effects of Zepbound® to be alert for:

  • Nausea
  • Diarrhea
  • Vomiting
  • Constipation
  • Stomach pain or discomfort
  • Indigestion
  • Feeling tired
  • Heartburn

If any of these linger or become hard to manage, it’s worth checking in with your provider. They may suggest staying at your current dose a little longer or adjusting your care plan to keep things manageable.

Serious side effects to be aware of

While most people tolerate Zepbound® well, there are some serious side effects you should be aware of. These are rare, but it’s important to know the signs so you can act quickly if something feels wrong. If you experience any of these, get immediate medical attention. Here are symptoms to look out for:

  • Pancreatitis: Be alert for persistent stomach pain, especially if it radiates to your back.
  • Allergic reactions: Symptoms include swelling of the face, lips, tongue, or throat; difficulty breathing or swallowing; severe rash or itching; dizziness; or a rapid heartbeat.
  • Vision changes: Get any sudden vision changes checked immediately.
  • Low blood sugar (hypoglycemia): If you have diabetes and use medications like insulin or other blood sugar-lowering drugs, watch for symptoms like dizziness, shakiness, sweating, or confusion.
  • Kidney problems: Symptoms like swelling, fatigue, shortness of breath, or changes in urination could signal kidney issues, especially if dehydration occurs from nausea, vomiting, or diarrhea.
  • Thyroid tumors: Watch for lumps or swelling in your neck, hoarseness, difficulty swallowing, or shortness of breath. Animal studies have shown a potential link between tirzepatide and thyroid tumors, but these types of tumors haven’t been reported in people.
  • Depression or suicidal thoughts: If you notice new or worsening mental health symptoms, seek help immediately.

Zepbound: Overdose risks and symptoms

Taking more Zepbound® than prescribed can lead to stronger or longer-lasting side effects than usual. If you accidentally take an extra dose or inject more than directed, you may notice symptoms like:

  • Intense nausea or vomiting that won’t let up
  • Ongoing diarrhea or sharp stomach pain
  • Feeling extremely tired, dizzy, or confused
  • Shakiness, a racing heart, or blurred vision
  • Signs of low blood sugar, like sweating or trouble concentrating

Because Zepbound® stays in your system for several days, your care team might need to monitor and manage your symptoms for a little while to make sure you stay stable and comfortable.
