Wegovy® and Saxenda® are both GLP-1 receptor agonists. What this means is that their active ingredients mimic the GLP-1 hormone. Wegovy® vs. Saxenda®: Who can take them?

  • Both medications are approved for adults with a BMI of 30 or higher or a BMI of 27 or higher if they also have a weight-related condition, such as high blood pressure, type 2 diabetes, or high cholesterol. Find out what your BMI is here.
  • Wegovy has also been approved to reduce cardiovascular risks in adults with heart disease who have the same BMI as stated above.

Wegovy® vs. Saxenda®: How are they taken?

Wegovy® and Saxenda® are both injected into the upper arm, thigh, or belly using a pre-loaded injector pen. However, the frequency and dosing are different. The biggest difference is that Wegovy® only requires a weekly injection, while Saxenda® must be injected once per day.

  • Wegovy®:
    • Schedule: Once-weekly injection 
    • Starting dose: 0.25 mg once per week. Gradually increased over time to 0.5 mg, 1.0 mg, 1.7 mg, and then 2.4 mg (the maximum dose)
  • Saxenda®:
    • Schedule: Daily injection
    • Starting dose: 0.6 mg once per day. Gradually increased to 1.2 mg, 1.8 mg, 2.4 mg, and finally 3.0 mg (the maximum dose)

When a manufacturer reports a drug presentation as “available,” it means they are releasing supplies, but the FDA clarifies that this doesn’t necessarily indicate the shortage has been resolved. Availability can still vary significantly depending on your pharmacy and location.

  • Nausea: The most common side effect for both medications, often mild to moderate. It’s more likely when you first start the medication or increase your dosage, but it usually fades over time.
  • Diarrhea: You may experience loose or frequent stools as your digestive system adapts to the medication.
  • Constipation: Some people experience slower digestion, which can lead to constipation. Staying hydrated and eating fiber-rich foods can help.
  • Vomiting: Some may experience occasional vomiting, especially during the early stages or dose increases.
  • Abdominal pain: Mild to moderate discomfort or cramping in the stomach area can occur, affecting around 20% of users.
  • Headaches: Some users report headaches during the first few weeks of treatment.
  • Injection site reactions: Mild redness, swelling, or irritation at the injection site is possible but generally resolves quickly.
  • Fatigue: Both medications can cause tiredness, especially during the initial weeks of treatment.
  • Low blood sugar (hypoglycemia): This is more common in people with diabetes who take glucose-lowering medications. Symptoms include shakiness, sweating, and confusion.
  • Increased heart rate: Both medications may cause an increase in heart rate or palpitations.

Serious side effects and warnings

  • Pancreatitis: Symptoms include severe abdominal pain that may radiate to your back, along with nausea and vomiting.
  • Gallbladder issues: These include gallstones or gallbladder infections. Watch for upper-right abdominal pain, nausea, fever, or yellowing of the skin or eyes (jaundice).
  • Kidney problems: Dehydration from nausea, vomiting, or diarrhea can lead to decreased urination, swelling in the legs or feet, and fatigue.
  • Allergic reactions: Seek immediate medical attention if you experience a rash, itching, swelling of the face or throat, or difficulty breathing.
  • Vision changes: Blurry vision or sudden changes in eyesight may occur, especially in individuals with diabetes-related eye conditions (like diabetic retinopathy).
  • Thyroid tumors: Both medications carry a warning about a potential risk of thyroid C-cell tumors based on animal studies. Watch for unusual lumps or swelling in the neck or difficulty swallowing.

Who should avoid Wegovy® or Saxenda®?

  • Are pregnant, trying to conceive, or breastfeeding: These medications are not for people who are pregnant, trying to conceive, or breastfeeding.
  • Have a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2): These conditions are associated with a higher risk of thyroid tumors.
  • Have a history of pancreatitis: If you’ve had inflammation of the pancreas, these medications may increase your risk of recurrence.
  • Have severe kidney or liver disease: Avoid these medications if you have significant kidney or liver impairment.
  • Have gastroparesis (delayed stomach emptying): These medications can slow digestion further and worsen symptoms.
  • Have type 1 diabetes: Neither Wegovy® nor Saxenda® is approved for managing type 1 diabetes.
