How to inject Zepbound®: Differences between single-dose pen, vial, and KwikPen®

What you’ll learn:

  • Zepbound® single-dose pens, multi-dose KwikPens®, and vials contain the same medication but differ in how you prepare the dose, handle the needle, and give the injection.
  • All three forms of Zepbound® are injected once weekly under the skin and can be given in the abdomen, thigh, or back of the upper arm.
  • Which Zepbound® device you get can depend on where you fill your prescription, your insurance or self-pay option, and whether you qualify for programs like the Medicare GLP-1 Bridge.

You probably know that Zepbound® can be highly effective for weight loss and that it’s taken as a weekly injection. But if you’ve never injected yourself before, you probably have some questions about the process: Will I know what I’m doing? Will I do it right? How do I know that I got the full dose?

Those are common questions, and the answer starts with which Zepbound® device you have. Zepbound® comes in three different forms: a single-dose auto-injector pen, a single-dose vial used with a syringe, and a multi-dose pen called a KwikPen®. The medication inside is the same, but the setup and the instructions you need to follow are different.

Which one you get can also depend on how you pay for Zepbound® and where you fill your prescription.

  • The single-dose pens are the version you’ll typically find at retail pharmacies and may be covered by insurance.
  • The vials are available as a self-pay option through LillyDirect, the company’s direct-ship pharmacy.
  • The multi-dose KwikPen® is available through LillyDirect for people paying cash or using insurance and is also the version used by the Medicare GLP-1 Bridge Program.

Once you know which device you have access to, the injection itself comes down to learning a few specific steps. You’ll also need to choose an approved injection site and rotate where you inject each week to help minimize irritation.

Below, we’ll walk through how the single-dose pen, KwikPen®, and vial work, where and how to inject each one, and access differences that may help determine which option you use.

How to inject Zepbound: Understanding the single-dose pen, multi-dose KwikPen, and vial

Zepbound comes as a single-dose auto-injector pen, a single-dose vial used with a syringe, or a multi-dose KwikPen®. That matters because each one has different instructions for how to inject it.

No matter which type you have, Zepbound® is injected once a week under the skin (subcutaneously), not into a muscle or vein. But how much you interact with the needle varies quite a bit.

With the single-dose pen, the needle is built in and stays hidden—you never have to attach or see it. With the KwikPen®, you attach a small pen needle before each weekly dose and remove it afterward. And with the vial, you use a traditional syringe and needle to draw up the medication and give yourself the injection.

So, if needles make you nervous, the single-dose pen may feel very different from using a vial and syringe. The KwikPen® falls somewhere in between: You handle the needle, but you don’t have to draw medication out of a vial.

Here’s what makes the three delivery options different:

Zepbound® optionWhat is it?What do you do?What about the needle?
Single-dose penA prefilled device that gives you one weekly dose.Place the pen against your skin, unlock it, and press a button. The pen gives the injection automatically, and you throw it away after use.The needle is built into the pen and stays hidden. You don’t need to attach or handle it.
KwikPen®A reusable pen that contains four weekly doses.Attach a new needle each week, prepare the pen, and press the dose knob to give yourself the injection. You will keep the pen for 4 doses. You attach a small pen needle before each injection, remove it afterward, and throw the needle away.
Single-dose vialA small vial that contains one weekly dose.Use a syringe to draw the medication out of the vial, then give yourself the injection. Both are thrown away after each dose. You handle the syringe and needle yourself.

Choosing a Zepbound injection spot: Where to inject for the best results

Another choice you have when giving yourself Zepbound injections is where to inject. Zepbound® can be injected into the abdomen, thigh, or the back of the upper arm.

The important thing to know is that there isn’t one injection site that produces better weight-loss results. What matters most is choosing an approved site that’s comfortable and easy to reach, then rotating where you inject each week.

Choose a soft, fatty area just under the skin—not muscle. You can use the same general area each week, but move at least 1 inch from your last injection spot. Avoid skin that is tender, bruised, hard, scarred, lumpy, or irritated.

How to inject Zepbound into the thigh

Use the front of your thigh, between your hip and knee, choosing an area with enough fatty tissue to inject just under the skin. Don’t inject into the inner or outer thigh, the back of the thigh, or close to your knee. Avoid areas that are bruised, tender, hard, or irritated.

💡 Tips: Sit down while you inject so your leg is relaxed, and you can clearly see the injection site. Switch thighs or choose a different spot on the same thigh each week rather than repeatedly injecting in one place.

How to inject Zepbound into the stomach

Choose an area on your abdomen that’s at least two inches away from your belly button. You have a relatively large area to work with, which can make the abdomen convenient for rotating injection spots.

💡 Tips: Pick a spot you can easily see and reach without twisting. Let the alcohol from your swab dry before injecting, and avoid injecting through clothing or into scars, irritated skin, or areas where a waistband will rub against the injection site.

How to inject Zepbound into the arm

Use the back of the upper arm, where there is fatty tissue under the skin. Because this area can be difficult to see and reach correctly yourself, another person should give you the injection.

💡 Tips: Have the person giving your injection choose the spot while your arm is relaxed. Alternate arms or move to a different spot on the back of the upper arm the next time you use this injection area. Don’t try to twist around to give yourself an upper-arm injection.

How to inject Zepbound® with a single-dose pen

The single-dose auto-injector pen comes prefilled with one weekly dose. It has a hidden, built-in needle, so there is no measuring or needle attachment. You use it once and then discard it.

Supplies

  • One single-dose Zepbound® pen
  • One alcohol swab
  • Gauze or a cotton ball
  • An FDA-cleared sharps container

Steps

  1. Check the pen label and expiration date. Keep the gray base cap on until you are ready to inject. The liquid should be clear and colorless to slightly yellow; air bubbles are normal.
  2. Wash your hands with soap and water.
  3. Choose and clean an approved injection site. Let the skin dry completely.
  4. With the pen still locked, pull the gray base cap straight off and throw it away. Do not touch the needle end of the pen.
  5. Place the clear base flat against your skin, then unlock the pen by turning the lock ring.
  6. Press and hold the purple injection button for up to 10 seconds. The first click means the injection has started; the second means it is complete. The gray plunger should be visible in the window.
  7. Remove the pen. If you see a small drop of blood or liquid, press gently with gauze or a cotton ball. Do not rub the site.
  8. Put the entire used pen into a sharps container right away. Never reuse a single-dose pen.
  9. A brief sting, mild redness, tenderness, or a small bruise at the injection site can happen and usually fades within a day or two. Rotating your injection spot each week can help reduce irritation.

How to inject Zepbound® with the KwikPen®

The KwikPen® is a multi-dose pen that contains four premeasured weekly doses. Unlike the single-dose pen, it doesn’t have a built-in needle. You attach a new compatible pen needle each week and keep the pen until all four doses have been used.

Supplies

  • One Zepbound® KwikPen®
  • One new compatible pen needle for each injection
  • One alcohol swab
  • Gauze or a cotton ball
  • An FDA-cleared sharps container

Steps

  1. Check the pen label and expiration date. The medicine should be clear and colorless to slightly yellow. Do not use it if it is frozen, cloudy, discolored, or has particles.
  2. Wash your hands and choose an approved injection site. Clean the skin with an alcohol swab and let it dry.
  3. Attach a new compatible pen needle. Never reuse a needle or share your KwikPen® with anyone.
  4. Prime the pen and dial the dose according to the Instructions for Use that come with your prescription.
  5. Insert the needle into the cleaned skin, then push the dose knob all the way in.
  6. Keep the dose knob pushed in and hold the needle in place for a slow count of five before removing it.
  7. Remove the pen needle and place it in a sharps container. Never store the KwikPen® with a needle attached.
  8. Put the pen cap back on and store the KwikPen® as directed. After the fourth dose, discard the entire pen, even if medication appears to remain.

How to inject Zepbound® with a syringe and vial

A single-dose vial contains one weekly dose that you draw into a syringe yourself. It takes a few more preparation steps than either pen, but the medication works the same way. Have your prescriber or pharmacist demonstrate the process before your first injection.

Supplies

  • One single-dose Zepbound® vial
  • One syringe with its needle attached
  • One alcohol swab
  • Gauze or a cotton ball
  • An FDA-cleared sharps container

Steps

  1. Check the vial label, strength, and expiration date. The medicine should be colorless to slightly yellow and free of particles. Do not use it if it is frozen, cloudy, or discolored.
  2. Wash your hands. Remove the vial’s plastic cap, leave the rubber stopper in place, and wipe the stopper with an alcohol swab.
  3. Open the syringe and remove the needle shield. Pull air into the syringe until the plunger tip reaches the 0.5 mL mark.
  4. Insert the needle through the vial’s rubber stopper and push the plunger all the way down to put the air into the vial.
  5. Turn the vial and syringe upside down. Keep the needle tip in the liquid and pull the plunger down a little past the 0.5 mL mark.
  6. Gently tap out air bubbles, then slowly push the plunger until it lines up exactly with the 0.5 mL mark.
  7. Choose and clean an approved injection site. Insert the needle as your care team showed you, push the plunger down fully, and keep the needle in place for at least five seconds.
  8. Remove the needle. Press gently with gauze if needed, but do not rub or recap the needle.
  9. Put the used syringe, needle, and vial into a sharps container right away. Never reuse them. A small drop of blood after you remove the needle is usually normal.

Choosing between a single-dose pen, KwikPen®, and vial

The medication works the same way in all three formats. The practical differences are whether you want a hidden needle, how much setup feels comfortable, what your insurance covers and what you’d like to pay, and which device is available to you.

  • Single-dose pen: May be a good fit if you want an automated injection with a hidden needle and can pay full price or have insurance coverage.
  • KwikPen®: May be more convenient if you prefer four weekly doses in one device and are comfortable attaching a pen needle. If you qualify for the Medicare GLP-1 Program or want to pay cash through LillyDirect, this will be an option.
  • Vial: May be a good fit if you’re comfortable drawing up medication with a syringe. This is also an option if paying cash through LillyDirect.

Cost can be a big factor in your decision. What you’ll pay will depend on your insurance and eligibility for certain programs.

Frequently asked questions about how to inject Zepbound®

Is there a video showing how to inject Zepbound®?

Yes. Lilly has step-by-step videos for the single-dose pen, single-dose vial, and KwikPen on its how to use Zepbound page. Watch the video that matches the exact device on your prescription.

How do you know whether you injected Zepbound® correctly?

  • With the single-dose and KwikPen, you’ll usually hear two clicks and see the gray plunger in the window when the injection is complete.
  • With a vial and syringe, you should push the plunger all the way down and keep the needle in place for at least five seconds. A tiny drop of blood or liquid afterward doesn’t necessarily mean the dose was missed.

If the pen is damaged, the medication leaks before or during injection, the pen doesn’t complete its cycle, or you aren’t sure you received the dose, call your pharmacist or prescriber. Don’t inject an extra dose unless they tell you to.

Noom GLP-1rx Plus — tirzepatide

How long does a Zepbound® injection take to kick in?

The injection itself takes seconds with a pen and a little longer with a syringe. Inside the body, tirzepatide begins absorbing after the injection, but how soon you notice appetite changes or weight changes varies. Zepbound is designed to be taken weekly and built up gradually according to your prescribed dosing schedule, so it’s not an instant-result medication.

Can you use the same Zepbound® injection site every week?

You can use the same general area, but rotate to a different spot each week. Injecting into the exact same place repeatedly or into irritated or damaged skin can increase the risk of injection site reactions.

What if I’m nervous about injecting Zepbound®?

That’s very common. Ask your care team for a hands-on demonstration, watch the official device video, and plan your first injection when you don’t feel rushed. If seeing a needle makes you anxious, the single-dose pen keeps the needle hidden. If you have a vial, ask your pharmacist to review the draw-up and injection steps with you before you leave.

The bottom line: How you inject Zepbound® depends on your device

Zepbound® is always injected once weekly under the skin, but the steps depend on which device you have.

Whichever version you use, check the instructions for your specific device before your first dose, choose an approved injection site, and rotate your injection spot each week. If you’re unsure whether you prepared or injected a dose correctly, contact your prescriber or pharmacist rather than giving yourself another dose.

If you’re wondering if Zepbound® or another GLP-1 medication is right for you, see if you qualify for Noom Med.
