Wegovy® dosage chart: Weekly injection, Wegovy® HD, and daily pill

What you’ll learn:

  • Wegovy® starts at a low dose and gradually increases, with different schedules for the weekly injection and daily pill.
  • Wegovy® HD is the highest injection dose, but moving up depends on how you tolerate treatment and whether you need additional weight loss.
  • Missed doses, switching forms, and managing side effects each require guidance specific to the form you take.

Wegovy® is one of the most effective weight loss medications available. With a weekly injection, a higher-dose option called Wegovy® HD, and a daily pill, there are now more ways to fit treatment into your life. But to get the most benefit and minimize side effects, finding the dose that works best for you is key.

Both the injection and pill follow a gradual dose escalation schedule—starting low and slowly increasing over time—to give your body time to adapt. The injection starts at 0.25 mg once weekly, while the pill starts at 1.5 mg once daily. This process, called titration, helps reduce uncomfortable side effects like nausea that can happen when you first start the medication or when your dose is increased. This slower approach can also make it easier to stay on treatment, since side effects are one of the reasons people stop taking weight loss medications.

We’ll walk you through everything you need to know about Wegovy® dosing, including how each step-up schedule works, where Wegovy® HD fits in, and how costs vary by form and dose. We’ll also explain how your provider can personalize your treatment based on your response and why the highest dose isn’t necessary for everyone. Plus, you’ll learn about important safety considerations, managing side effects, switching between forms, and what to do if you miss a dose.

What is Wegovy®? The injection, Wegovy® HD, and pill explained

Wegovy® is an FDA-approved prescription medication that helps people lose weight and keep it off. It contains semaglutide, a medication that mimics GLP-1, a hormone your body naturally produces to help regulate appetite and blood sugar.

Wegovy® works in several ways:

The Easy Way

to lose weight and get healthy.

*Initial 3 week subscription and 4 weeks of medication from $79 plus tax and $199 per month plus tax for 12 week subscription thereafter. New pricing for new accounts only effective as of March 31, 2026.

  • Reduces hunger and cravings: It acts on areas of the brain involved in appetite, helping you feel less hungry and more satisfied with smaller portions.
  • Helps you feel full longer: It slows how quickly food leaves your stomach, which can help you stay satisfied between meals.
  • Helps regulate blood sugar: It stimulates insulin release when blood sugar is elevated and reduces the amount of glucose your liver produces.

Together, these effects can make it easier to eat less without feeling as hungry, supporting gradual, sustained weight loss.

What forms does Wegovy® come in?

Wegovy® is available in two forms with several different strengths, giving people different ways to take the medication:

  • Wegovy® injection: A once-weekly injection administered with a prefilled, single-dose pen. The standard maintenance doses are 1.7 mg and 2.4 mg. A higher-dose option, called Wegovy® HD, delivers 7.2 mg weekly and may help eligible adults achieve additional weight loss.
  • Wegovy® pill: A once-daily oral version of semaglutide that offers a needle-free alternative. It works the same way as the injection but has its own dosing schedule and must be taken on an empty stomach with a small amount of water.

Both forms are approved for weight management in eligible adults and to reduce the risk of major cardiovascular events, such as heart attack or stroke, in adults with established cardiovascular disease and overweight or obesity. The injection also has approvals for adolescents ages 12 and older with obesity and for adults with noncirrhotic metabolic dysfunction–associated steatohepatitis (MASH) with moderate to advanced liver scarring.

Regardless of which form you use, Wegovy® treatment starts at a low dose that gradually increases. Higher doses can lead to greater weight loss, but they may also increase side effects. Following the recommended dosing schedule helps your body adjust, making treatment more tolerable and helping you stay consistent over time.

Wegovy® dosage chart: Every FDA-approved dose at a glance

Whether you take Wegovy® as a weekly injection or a daily pill, you’ll start with a low dose and gradually work your way up. The chart below shows the adult dosing schedule from the starting dose to maintenance. While the timeline suggests you can increase after 4 weeks (or 30 days for the pills), that’s just a minimum. Your provider may suggest staying at a lower dose for longer to help with reducing side effects or if your weight loss is optimal.

FormDose progression
Wegovy® injection0.25 → 0.5 → 1 → 1.7 → 2.4 mg → 7.2 mg (if needed)
Wegovy® pill1.5 → 4 → 9 → 25 mg

These schedules follow the manufacturer’s dosing guidance, but your provider may recommend spending more time at a particular dose if you’re having side effects. And some people never need to reach the highest dose to get their intended weight loss results. Always check with your prescriber before making any changes.

Wegovy® injection dosing: From 0.25 mg to 7.2 mg

If you’re taking the weekly injection, your provider will usually increase your dose every four weeks. Starting low gives your body time to adjust before you move to the next dose, which can help make digestive side effects more manageable.

Treatment periodDose once weekly
Weeks 1–40.25 mg
Weeks 5–80.5 mg
Weeks 9–121 mg
Weeks 13–161.7 mg
Weeks 17–202.4 mg if needed
Weeks 20 and onward7.2 mg if needed and approved

Your provider will use this injection schedule to guide treatment, while keeping an eye on how you feel. If side effects make an increase difficult, they may recommend waiting another four weeks before moving up.

Four weeks is only the minimum time to wait before increasing doses, and some people never need to increase to the full 2.4mg.

Wegovy® HD 7.2 mg: When a higher dose may help

The FDA approved Wegovy® HD on March 19, 2026. It offers another option for adults who may benefit from additional weight loss after reaching the standard maintenance dose.

Your provider may recommend Wegovy® HD if:

  • You’ve tolerated 2.4 mg weekly for at least four weeks.
  • You have additional weight to lose, and your provider determines that a higher dose is appropriate.

Not everyone needs to move up to HD. If your current dose is helping you make progress and feels manageable, your provider may recommend continuing it.

In the published STEP UP trial, adults with obesity without diabetes lost an average of 18.7% of their starting weight with 7.2 mg, compared with 15.6% with 2.4 mg, at 72 weeks. These estimates included people who stopped treatment. All groups received lifestyle support.

A higher dose can also come with more side effects. In the trials summarized in the prescribing information, 22% of people taking 7.2 mg reported unusual skin sensations compared with 6% taking 2.4 mg. These symptoms, called dysesthesia, can include tingling, burning, or increased skin sensitivity. Let your provider know if you notice them so you can discuss what to do next.

Wegovy® pill 25 mg: The daily oral option

If a daily pill fits your routine better than a weekly injection, the Wegovy® pill offers another way to take semaglutide. Like the injection, it starts at a low dose and gradually increases. The main difference is that you take it every morning and follow specific instructions around food, drinks, and other medications.

Treatment periodDose once daily
Days 1–301.5 mg
Days 31–604 mg
Days 61–909 mg
Day 91 onward25 mg if needed

Why is the pill dose higher than the injection dose?

Only a small portion of oral semaglutide reaches the bloodstream. The pill contains an absorption enhancer called SNAC to help semaglutide pass through the stomach lining, but absorption still differs substantially from an injection.

This helps explain why the pill’s maintenance dose is 25 mg daily, while the recommended injection maintenance dose is 2.4 mg weekly. Your body absorbs the two forms differently, so the milligram numbers don’t tell you which one will work better for you.

How much weight can you lose with the pill?

In OASIS 4, adults taking oral semaglutide 25 mg lost an average of 13.6% of their starting weight at 64 weeks, compared with 2.2% with placebo. Participants had overweight or obesity without diabetes, and both groups received lifestyle interventions.

The study didn’t compare the pill directly with the injection. Your provider can help you consider the research alongside your preferences, side effects, and daily routine when choosing between them.

Increasing your Wegovy® dose: When to move up or wait

Finding the right Wegovy® dose is a gradual process. Your provider will consider how well the medication is working and how you’re feeling before recommending an increase. Keeping them updated can help you make progress while keeping side effects manageable.

Before an increase, tell your provider about:

  • Digestive symptoms: Nausea, vomiting, constipation, or diarrhea that interferes with your day.
  • Eating and drinking: Difficulty getting enough food or keeping fluids down.
  • Your progress: Changes in appetite, weight, energy, and other health goals.
  • Medication changes: New prescriptions or adjustments to medicines you already take.

If you’re having side effects with the injection or pill, your provider may recommend waiting another four weeks before increasing your dose.

Wegovy® dosage adjustments: Finding the dose that works for you

The best Wegovy® dose is the one that supports your health goals while keeping side effects manageable. Your provider will consider your progress, medical history, and response to treatment when helping you choose a maintenance dose. The highest dose may be helpful for some people, but it isn’t necessary for everyone.

Keeping a simple record of your dose, appetite changes, and side effects can make those conversations easier. Let your provider know if you’re struggling to eat enough, feeling unwell, or finding it hard to keep up with your medication routine. These details can help them adjust your treatment to better fit your needs.

How Wegovy® side effects may change by dose

Wegovy® side effects can vary depending on the dose and whether you take the daily pill or weekly injection. Digestive symptoms are common with both, while other effects may be more noticeable at higher doses.

Nausea was the most commonly reported side effect for each option, with vomiting, constipation, and other digestive symptoms also occurring frequently. Unusual skin sensations, called dysesthesia, were reported more often in the trials evaluating Wegovy® HD.

Your experience may look different from these averages. Let your provider know how you’re feeling, especially after a dose increase, so they can help you manage symptoms and decide whether your dose or form needs adjusting.

The table below shows reported side effect rates for the 25 mg pill, 2.4 mg injection, and Wegovy® HD 7.2 mg injection, based on the OASIS 4 trial and Wegovy® prescribing information. Because the results come from different trials and groups of people, they can help you understand what to expect, but they aren’t direct comparisons.

Wegovy® safety and serious side effects

Serious side effects from Wegovy® are uncommon, but they’re important to know about. Unlike common side effects, such as nausea and vomiting, which may become more noticeable as your dose increases, serious side effects aren’t known to be tied to dose. These safety warnings apply throughout treatment.

According to the Wegovy® prescribing information, potentially serious side effects and safety risks include:

  • Pancreatitis: Severe, persistent abdominal pain, sometimes spreading to your back, may be a sign of pancreatitis.
  • Gallbladder problems: Symptoms can include upper abdominal pain, fever, or yellowing of the skin or eyes.
  • Kidney problems: Persistent vomiting or diarrhea can cause dehydration, which may lead to kidney injury.
  • Severe allergic reactions: Swelling of the face, lips, tongue, or throat, or difficulty breathing requires emergency medical attention.
  • Low blood sugar: The risk is higher if you also take insulin or certain diabetes medications, such as sulfonylureas.
  • Vision problems: Wegovy® may worsen diabetic retinopathy in people with type 2 diabetes. Report sudden vision changes or vision loss immediately.
  • Severe digestive problems: Wegovy® can cause severe gastrointestinal reactions and isn’t recommended for people with severe gastroparesis.
  • Increased heart rate: Wegovy® can increase resting heart rate. Tell your provider if you notice a racing or pounding heartbeat.
  • Aspiration during anesthesia: Wegovy® slows stomach emptying, which may increase the risk of stomach contents entering the lungs during anesthesia or deep sedation.

Wegovy® also carries a boxed warning for thyroid C-cell tumors observed only in animals. It’s unknown whether Wegovy® causes these tumors in humans. It shouldn’t be used by people with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2).

Tell your provider about other medications you’re taking, any upcoming procedures involving anesthesia or deep sedation, and whether you’re pregnant, planning pregnancy, or breastfeeding. Wegovy® should be stopped when pregnancy is recognized for weight management and at least two months before a planned pregnancy. Breastfeeding is not recommended while taking Wegovy® tablets.

Missed a Wegovy® dose? What to do for each form

Missing a dose can happen, even with a consistent routine. What to do next depends on whether you take the weekly injection or daily pill—and, for the injection, how close you are to your next scheduled dose.

FormIf you miss one doseIf you miss multiple doses
Wegovy® injectionIf your next scheduled dose is more than 48 hours away, take the missed dose as soon as possible. If it’s less than 48 hours away, skip it and take the next dose on your regular day. After two or more consecutive missed doses, contact your provider. Your provider will guide you through restarting at a lower dose and gradually increasing again to help reduce digestive side effects.
Wegovy® pillSkip the missed dose and take your prescribed dose the following morning. Don’t double up.Ask your provider how to restart after a longer interruption, particularly if you previously had side effects.

These instructions follow the current prescribing information. If you’re exactly at the 48-hour boundary, ask your pharmacist or provider before deciding whether to inject.

If you take an injection late, check with your provider or pharmacist about when to take the next one. They can help you keep enough time between doses and return to your usual routine. Avoid taking extra medication to make up for a missed dose.

Switching between different Wegovy® forms

You may find that a daily pill fits your life better than a weekly injection, or that you’d prefer the flexibility of taking an injection without planning around breakfast. If you’re considering a change, your provider can help you switch safely and explain which dose to start and when.

The manufacturer describes these switching schedules:

Current treatmentNew treatmentWhen to start
2.4 mg weekly injection25 mg daily pillOne week after the last injection
25 mg daily pill2.4 mg weekly injectionThe day after the last pill
25 mg daily pill that isn’t toleratedConsider 1.7 mg weekly injectionYour provider plans the transition

The FDA label’s direct switch applies to adults already taking Wegovy® 2.4 mg. If you’re on a lower injection dose and want to move to the pill, ask your prescriber which pill dose and timing make sense for you.

Instructions for taking Wegovy®

Taking Wegovy® as directed can help you get the most benefit from treatment. Along with following your dosing schedule, building a routine around your weekly injection or daily pill can make it easier to stay consistent.

Taking the weekly injection

Choose the same day each week. You can inject at any time of day, with or without food, under the skin of your abdomen, thigh, or upper arm. Rotate the exact spot with each dose.

Before your first injection, ask your provider or pharmacist to show you how to use the device you receive. Wegovy® has different approved injection devices, including single-dose pens, single-dose syringes, and a multidose FlexTouch® pen. The steps can differ, so follow the instructions that come with your prescription.

For the single-dose autoinjector pen, follow the manufacturer’s instructions: check the medicine, prepare the site, press the pen firmly against your skin, and keep it in place until the yellow bar stops moving. Dispose of the used pen safely in a sharps container. Use each single-dose pen once, and take the full prescribed dose.

Instructions for taking the Wegovy® pill

It’s important to follow these daily administration instructions to make sure the oral medication can be properly absorbed:

  1. Take it in the morning on an empty stomach.
  2. Use plain water only, up to 4 ounces.
  3. Swallow the tablet whole. Don’t crush, split, chew, or dissolve it.
  4. Wait at least 30 minutes before food, other drinks, or other oral medicines.

Following these exact timing instructions may be hard for someone, but the timing helps your body absorb the medication as intended. Once you swallow the Wegovy® pill, it needs some time alone in your stomach. The 30-minute wait gives the tablet time to dissolve and semaglutide time to begin being absorbed before food, drinks, or other medications enter your stomach and potentially interfere with that process.

Storing your medication

For single-dose Wegovy® and HD pens, refrigerate at 36–46°F in the original carton. Before removing the cap, they may be kept at 46–86°F for up to 28 days. Protect them from light and never use a pen that has frozen. Follow the storage instructions supplied with other injection devices.

For the Wegovy® pills, Keep tablets in their original, tightly closed bottle, protected from moisture, and at room temperature. Avoid moving them into a weekly pill organizer.

Wegovy® cost: The cheapest options per dose with and without insurance

Whether your cost increases with dose depends on insurance coverage and whether you take the pill or injection.

  • If you have insurance through a job or private and Wegovy is covered, you will not pay more per dose. But in most cases, insurance is rarely accepted when a medication is used for weight loss.
  • If you get the Wegovy pill or injection through the Medicare GLP-1 Bridge Program, your cost will not increase as the dose increases.
  • If you get Wegovy through the cash-pay NovoCare Pharmacy or a pharmacy that partners with it, you may pay more for higher doses.

With insurance: If your commercial plan covers Wegovy®, the savings card may lower your cost to as little as $25 a month, with savings capped at $100 monthly. If you’re covered by Medicare Part D, the Medicare GLP-1 Bridge Program can bring monthly costs to only $50 for any form of Wegovy®.

Without insurance coverage: NovoCare’s cash-pay programs are the cheapest option:

  • Wegovy® pill: $149 monthly for 1.5 mg, $199 for 4 mg, and $299 for 9 or 25 mg.
  • Wegovy® injection: Eligible new participants pay $199 per monthly fill for the 0.25 mg and 0.5 mg starter doses, for up to two fills through December 31, 2026. Standard self-pay pricing is $349 monthly for doses through 2.4 mg and $399 for Wegovy® HD 7.2 mg. NovoCare®

You’ll need a prescription, and insurance can’t be used for cash-pay purchases.

Frequently asked questions about Wegovy® dosing

What’s the starting dose of Wegovy®?

The injection starts at 0.25 mg once weekly for four weeks. The pill starts at 1.5 mg once daily for 30 days. These are introductory doses that help your body adjust before you move toward maintenance. Some people notice effects early, but that doesn’t mean the starter dose is an approved long-term maintenance dose.

What’s the highest Wegovy® dose?

For adults taking the injection for weight management, the maximum is 7.2 mg weekly as Wegovy® HD. Your provider may recommend it after you’ve tolerated 2.4 mg for at least four weeks, if you need additional weight loss. The pill’s maintenance and maximum dose is 25 mg daily.

How long do you stay on each Wegovy® dose?

The standard schedule increases the injection every four weeks and the pill every 30 days, until maintenance. Your provider may delay an increase because of side effects. Once you reach maintenance, you continue the prescribed dose unless your provider changes the plan.

What’s the difference between the Wegovy® shot and pill?

The injection is weekly and can be taken with or without meals. The pill is daily and requires an empty stomach, a limited amount of water, and a wait before breakfast, coffee, or other oral medicines. Both contain semaglutide, but their doses aren’t interchangeable milligram for milligram.

Does a higher Wegovy® dose cost more?

Not necessarily. If you have insurance, your cost may stay the same regardless of dose, depending on your plan. But if you’re paying out of pocket through NovoCare® Pharmacy’s self-pay program, some higher doses cost more than lower ones. Prices also vary between the injection, Wegovy® HD, and the pill. Check your expected cost before increasing your dose or switching forms.

The bottom line: The right Wegovy® dose is different for everyone

Finding the right Wegovy® dose takes time. Whether you take the weekly injection or daily pill, starting low and gradually increasing gives your body a chance to adjust while you and your provider see how treatment is working.

Remember: the dose that works for one person may not work for another. An individualized approach is more helpful than just following a chart, and not everyone needs to reach the highest dose. Your progress, side effects, and preferences all help your provider recommend a plan that fits your needs.

Staying consistent with your medication and building sustainable habits around food, movement, and sleep can help support long-term results. Keep your provider updated along the way, especially if side effects, missed doses, or changes in your routine make treatment harder to manage.

If you’re exploring medication for weight management, Noom can help you review treatment options with a clinician while building habits around nutrition, movement, and long-term health. Medication is prescribed when clinically appropriate, and medication costs are separate from program fees.
