Completing Your Online Surveys

Here is everything you need to know about completing the online surveys for the DYNAMO Study!

Starting a survey

Whenever it’s time to complete a timepoint for the study, you will receive a text message notification with a link to the survey, like this:

After you click on the link in the text message, the survey will open in your browser. There, you can see all of the required forms in the survey, and can complete them by clicking on the green “Let’s Go →“ button.

Uploading images to submit in the survey

For each timepoint in the study, you will need to submit 4 total images in the survey:

1. Your A1C value as shown on the test kit analyzer.

1. Your body weight as shown on the scale.

1. Your % body fat as shown on the scale.

1. Your % body water as shown on scale.

Receiving compensation

The last form in every survey will ask you to enter your email address. Your email address must match the email you used to sign up for the study, otherwise the research team will be unable to verify your Noom account.

In order to finish the survey, you will have to return to the survey to click the orange “I’m Done” button.

If you follow these steps, you should receive your Amazon.com gift card via email from noom@customer.rybbon.net within 1-2 business days! If you can’t find it, make sure to check your spam folder first.

Important things to know

• You can complete the forms in a survey in any order.

• Some parts of the survey are required in order for you to receive compensation, like uploading your A1C and scale photos. Others can be skipped at your discretion.

• In order to receive compensation, you must complete at least 75% of the questions in a survey. You will receive a notice at the end of the survey if you haven’t responded to at least 75% of the questions.

• You will not receive compensation if you don’t get to the end of the form and enter your email address!
