The goal is to find the dose that helps you lose steady weight or regulate your blood sugar with the fewest side effects.

How Ozempic® dosing works 

Ozempic® will be started at the lowest dose and gradually increased over time, based on your reaction to the medication. This approach allows your body to adjust and helps reduce side effects like nausea and other stomach issues. 

Starting dose: Whether you are taking Ozempic® for blood sugar control or weight loss, everyone starts at 0.25 mg once weekly. This dose will typically be continued for the first 4 weeks. This dose is intended to help your body adjust and reduce the risk of side effects that might be unmanageable. Some people do find that it works for weight loss, and they remain on it for a few months. 

Dose escalation schedule: After 4 weeks (or more if needed) at 0.25 mg, the dose is usually increased to 0.5 mg. Further increases (to 1 mg or 2 mg weekly) are considered if additional blood sugar control or weight loss is needed. These increases typically happen in monthly intervals and are part of the Ozempic® dosing schedule your provider will guide you through, but these can be stretched if you experience any uncomfortable side effects or if you’re losing weight steadily on a lower dose.

Maintenance dose: This is the dose at which you’re losing steady weight or your blood sugar is well-controlled. Ideally, you should have little to no side effects.

Maximum recommended dose: The highest dosage of Ozempic® currently approved by the FDA is 2 mg. If you need a higher dose of semaglutide to continue losing weight, you can switch to Wegovy®. This semaglutide medication has a maximum recommended dose of 2.4 mg and is FDA-approved for weight loss purposes.

Optimal dose for weight loss vs. diabetes

The ideal Ozempic® dose for you depends on what you’re using it for: diabetes or weight loss. 

If you’re managing type 2 diabetes, your provider will start you at the lowest dose and increase it slowly to the dose that works best to keep blood sugar in control, typically 0.5 mg or 1 mg once a week. These doses are typically effective in lowering blood sugar and can also result in some weight loss as a secondary benefit.

If you’re using Ozempic® for weight management, you will start the same way and increase to the dose that keeps you losing steady weight, about 1 to 2 pounds per week. This will typically be 1 mg or 2 mg. 

If you reach the maximum dose of Ozempic® for weight loss, you may need to switch to Wegovy®. Wegovy® is FDA-approved for weight loss at a higher dose than Ozempic®, but remember, it’s the same medication (semaglutide). 

How much weight can I expect to lose? 

Weight loss on Ozempic®  tends to increase with higher doses, but how much you lose depends on how you eat and the lifestyle changes you make. Here’s what one study found across different dosing levels:

  • 0.25 mg: This is the introductory dose used to help your body adjust to the medication. Weight loss is typically minimal at this stage, as it’s not intended for long-term use.
  • 0.5 mg and 1 mg: These are considered lower doses. People in this group saw an average weight loss of 5.1% at 3 months and 9.2% at 6 months.
  • 1.7 mg and 2.4 mg (with Wegovy®): These were the highest doses of semaglutide reached in the study. People taking these doses saw an average weight loss of 6.9% at 3 months and 12.1% at 6 months.

Everyone responds a little differently to medication, but this data shows a clear trend: the higher the dose (when tolerated), the greater the weight loss, especially with more time. 

How do you take Ozempic®?

You take Ozempic® once a week using a prefilled injection pen. You’ll inject the medication just under the skin (subcutaneously), usually into your abdomen, thigh, or upper arm.   • Before you start using a new pen: Keep it in the fridge between 36°F and 46°F (2°C to 8°C).
  • Once you’re using a pen: You can store it either at room temperature (59ºF to 86ºF) or back in the fridge—whatever’s easier for you.
  • Mark your calendar: After 56 days of use, toss the pen, even if there’s still some medication left inside.
  • Use a sharps container: Always put used pens and needles in an FDA-approved sharps container. Or, use a heavy-duty plastic container with a tight, puncture-proof lid, like a detergent bottle. Be sure to label it clearly to warn of hazardous waste.
  • Don’t freeze it: If Ozempic® freezes, you’ll need to throw it out—frozen medication isn’t safe to use.
  • Protect from heat and light: Keep your pen somewhere cool and out of direct light. (And in the fridge, don’t place it right next to the cooling element!)
  • Keep it capped: Always pop the pen cap back on when you’re not using it.

What happens if you miss a dose?
