If you’re managing type 2 diabetes, the focus is on improving blood sugar control. The starting dose will be 2.5 mg once a week. From there, the dose may gradually increase, depending on how well your blood sugar is responding and how you tolerate the medication.

If you’re using Mounjaro® for off-label weight management, you will start the same way (2.5 mg) and increase to the dose that keeps you losing steady weight with the fewest side effects. Many never reach the higher doses if the lower doses are working for them.

  • Thigh (front of the upper leg): The front outer part of your thigh, about halfway between your hip and knee, is a great option if you prefer to sit down while injecting.
  • Upper arm (back of the upper arm): The fleshy area on the back of your upper arm is another effective spot, though it can be a little trickier to reach on your own. If possible, have someone assist you with the injection to ensure it’s placed correctly and comfortably.

No matter where you inject, it’s important to rotate sites each week. This helps prevent common issues like skin irritation, bruising, or thickening.

  • Kidney problems: Diarrhea, nausea, and vomiting can cause dehydration and worsen kidney issues. Go to the ER if you experience decreased urination, swelling in the ankles or legs, fatigue, or shortness of breath.
  • Gallbladder problems: Symptoms include upper abdominal pain, fever, jaundice, or clay-colored stools. 
  • Allergic reaction: Symptoms include swelling of the face, lips, tongue, or throat, difficulty breathing or swallowing, or a rapid heartbeat. If you think you’re having an allergic reaction, get immediate medical attention. 
  • Thyroid tumors or cancer: Be alert for a lump or swelling in the neck, difficulty swallowing, or shortness of breath. (This connection has only been established in animal studies.)
